Under normal digestive physiology, the pyloric sphincter prevents retrograde movement of intestinal contents into the gastric compartment. When this sphincter fails to maintain adequate tone or when motility patterns become dysregulated, bile-laden fluid enters the stomach and triggers an inflammatory cascade that erodes the mucosal barrier. The condition is frequently identified in clinical practice under the more colloquial name bilious vomiting syndrome, though reflux gastritis syndrome encompasses a broader range of pathological changes beyond episodic vomiting alone.
The hallmark presentation of reflux gastritis syndrome involves intermittent vomiting of yellow or green bile-stained fluid, most commonly occurring in the early morning hours or after prolonged periods without food. This temporal pattern reflects the physiology of the interdigestive migrating motor complex, a cyclical pattern of gastrointestinal contractions that occurs during fasting and can promote duodenogastric reflux when the stomach is empty. While the condition is not typically life-threatening, chronic or severe cases can lead to significant mucosal damage, persistent discomfort, reduced appetite, and secondary complications including gastric ulceration and esophageal injury from repeated vomiting episodes.
Reflux gastritis syndrome can affect dogs of any age, breed, or size, though certain populations appear to experience the condition more frequently. Smaller breeds and highly active dogs seem to present with the syndrome at higher rates, and feeding schedules that involve long fasting intervals are a well-recognized contributing factor. The condition exists on a spectrum from mild, self-limiting episodes of occasional bilious vomiting to chronic inflammatory gastritis with persistent clinical signs that significantly impact quality of life. Understanding where an individual patient falls on this spectrum is essential for selecting appropriate management strategies.
Despite its prevalence, reflux gastritis syndrome is frequently underdiagnosed or dismissed as inconsequential because many affected dogs appear otherwise healthy between episodes. Owners may normalize occasional morning vomiting as a quirk of their dog's digestive system rather than recognizing it as a treatable medical condition. Veterinary attention is warranted when vomiting episodes occur regularly, as chronic exposure of the gastric mucosa to bile acids causes cumulative damage that worsens over time. Early recognition and appropriate intervention typically result in excellent control of symptoms and prevention of progressive mucosal injury.
