Mammary inflammatory carcinoma, also known as inflammatory mammary cancer or IBC, represents one of the most aggressive and devastating forms of mammary cancer in dogs. This highly malignant tumor type is characterized by rapid growth, extensive involvement of the skin and lymphatic vessels, and a distinctive inflammatory appearance that can initially be mistaken for mastitis or other non-cancerous conditions. Unlike typical mammary carcinomas that present as discrete lumps, inflammatory carcinoma causes diffuse swelling, redness, warmth, and pain affecting large areas of the mammary tissue and overlying skin. This cancer accounts for a small percentage of all mammary tumors but carries a significantly worse prognosis than other mammary cancer types.
The pathophysiology of inflammatory mammary carcinoma involves aggressive cancer cell proliferation combined with extensive invasion of dermal lymphatic vessels. This lymphatic invasion, called tumor emboli, blocks normal lymphatic drainage and causes the characteristic inflammatory appearance including skin thickening, pitting edema, and reddish discoloration. The cancer spreads rapidly through lymphatic channels to regional lymph nodes and commonly metastasizes to the lungs and other distant sites early in the disease course. By the time inflammatory carcinoma is diagnosed, the disease has typically already spread beyond the mammary tissue, limiting treatment options.
Inflammatory mammary carcinoma severely impacts affected dogs' quality of life from the time of diagnosis. The affected mammary tissue is typically painful, and the inflammation causes significant discomfort. Dogs may be reluctant to lie down, may show decreased appetite, and may exhibit signs of systemic illness including lethargy and fever. The rapid progression of the disease means that dogs often deteriorate quickly over weeks to months. The extensive nature of the cancer at presentation usually makes surgical removal impossible, and response to chemotherapy and radiation is generally limited.
The prognosis for dogs diagnosed with inflammatory mammary carcinoma is unfortunately poor, with most dogs surviving only weeks to a few months following diagnosis. Complete surgical excision is rarely possible due to the diffuse nature of the disease, and the cancer is often resistant to standard chemotherapy protocols. Treatment focuses primarily on palliative care to maintain comfort and quality of life for as long as possible. Early recognition of the signs of inflammatory mammary carcinoma is important to allow families time to make informed decisions about care and to initiate supportive treatment promptly.
