Esophageal stricture is a condition characterized by abnormal narrowing of the esophagus caused by the formation of scar tissue within the esophageal wall. This narrowing creates a physical obstruction that impedes the normal passage of food and water from the mouth to the stomach, resulting in regurgitation, difficulty swallowing, and potentially serious nutritional deficiencies. Strictures typically develop as a consequence of previous esophageal injury, with the body's healing response producing fibrous tissue that contracts and narrows the esophageal lumen. The condition can affect dogs of any age, breed, or size, with occurrence directly related to the underlying cause of esophageal damage.
The most common causes of esophageal stricture in dogs include damage from foreign body lodgement, severe gastroesophageal reflux, caustic substance ingestion, and complications following anesthesia. When foreign bodies become lodged in the esophagus, the pressure against the esophageal wall causes tissue injury that heals with scar formation. Gastroesophageal reflux, particularly when severe or prolonged, allows gastric acid to damage the esophageal lining, leading to inflammation and subsequent stricture formation. Caustic substances, whether acidic or alkaline, cause chemical burns that heal with significant scarring. Anesthesia-related strictures occur when reflux of gastric contents during procedures damages the esophagus, particularly in prolonged surgeries or when proper precautions are not taken.
The impact of esophageal stricture on affected dogs depends on the severity and location of the narrowing. Mild strictures may cause only occasional difficulty with larger food pieces, while severe strictures can prevent passage of even liquid diets. Dogs with significant strictures experience frequent regurgitation, progressive weight loss, and risk of aspiration pneumonia when regurgitated material enters the airways. The psychological impact of being constantly hungry while unable to retain food causes significant distress. Without treatment, severe strictures lead to malnutrition, dehydration, and life-threatening complications. The location of the stricture also influences symptoms and treatment approach, with thoracic strictures generally more challenging to manage than those in the cervical esophagus.
Treatment of esophageal stricture primarily involves mechanical dilation of the narrowed segment to restore adequate lumen diameter. Balloon dilation, performed endoscopically, is the most common treatment approach, using inflatable balloons passed through the stricture to stretch and break up scar tissue. Multiple dilation procedures are typically required, as strictures tend to reform between treatments until adequate scar tissue remodeling occurs. Medical management with medications to reduce inflammation, inhibit acid production, and prevent further esophageal damage accompanies dilation therapy. With appropriate treatment, many dogs achieve acceptable swallowing function and quality of life, though some require ongoing management and periodic dilation procedures.
